I'm assuming that you mean that you want to show multiple MC questions o a single display, right? (Otherwise you could just append multiple form_multiple_choice items to your trial sequence.)
If you are running your experiment locally, it sounds like you could create custom forms, either by using form_base items or some Python inline code. For more information, see:
If you are running your experiment online, questionnaires are a bit more tricky (since form items are not supported) but perhaps the template provided in this thread can be of help:
